Patient's Query
Hi doctor,
I am a 42-year-old woman who has been experiencing easy bruising all over my body, even from minor bumps, and tiny red spots on my skin that my doctor called petechiae. I have had several nosebleeds that were difficult to stop, and my menstrual periods have become much heavier and longer than normal.
Blood work showed my platelet count is dangerously low at 15,000 per microliter, and my hematologist suspects chronic immune thrombocytopenia. I am worried about having a serious bleeding episode, and I do not understand why my immune system would attack my own platelets.
Please help.
Thank you.
Hi,
Welcome to icliniq.com.
I have gone through your query and understand your concern.
Chronic immune thrombocytopenia (ITP) is an autoimmune condition in which your immune system destroys platelets (cells that help blood clot). This explains your low platelet count of 15,000 per microliter, bruising, petechiae, nosebleeds, and heavy periods. The main concern is preventing serious bleeding.
Treatment options include:
First-line treatment includes corticosteroids (Prednisone) and IVIG (intravenous immunoglobulin), which can quickly but temporarily raise platelets.
If the condition is persistent or relapsing, then thrombopoietin receptor agonists (Eltrombopag, Romiplostim), immunosuppressants (Rituximab), or, in some cases, splenectomy (removal of the spleen).
Lifestyle precautions:
Avoid contact sports, heavy lifting, and blood-thinning drugs like Aspirin or NSAIDs (non-steroidal anti-inflammatory drugs).
Seek urgent care for severe headache, blood in stool or urine, or uncontrolled bleeding.
With proper treatment, most patients achieve safe platelet levels and reduced bleeding risk. Regular hematology follow-up is essential.
I hope this clarifies your concern.
Thank you.
