Does Ajovy work better in the stomach than the thigh area?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 25-year-old and started Ajovy injections four months ago for chronic migraine. My headaches have reduced a lot, but I noticed something odd. Why does Ajovy at 25 work better when injected in the stomach than the thigh? When I inject it in my thigh, the migraines seem to creep back sooner. Stomach shots appear more effective.

Is absorption really different depending on the injection site? Does body fat play a role? Am I imagining this, or is it a common experience? I want to stick to the most effective method in the long-term. Also, does rotating injection sites reduce the benefit over time or not?

Kindly help.

Hello,

Welcome to icliniq.com

I have read your query and understand your concern.

I am sorry for the discomfort. Fremanezumab (Ajovy) injections work the same in all approved injection sites (stomach, thigh, and arm) according to studies.

However, in real life, the stomach (abdomen) may feel more effective for some people because

  • It usually has more consistent fat, so the injection goes in properly.

  • Absorption may be steadier.

  • It is often easier to inject correctly than in the thigh.

The thigh can sometimes give slightly less consistent absorption, especially if:

  • There is less fat or more muscle in the area.

  • The injection depth varies.

Therefore, your experience is possible and not uncommon. You are not imagining it. It is perfectly acceptable to prefer the stomach if you feel it works better for you. However, you should rotate injection sites within the stomach area and avoid injecting into the same spot each time.

If stomach injections give you more reliable migraine control, it is reasonable to stick with that area and rotate within it for the best long-term results.

I hope you find my reply helpful. For further queries, you can consult me at iCliniq.

Thankyou.
