Patient's Query
Hello doctor,
My age is 59 years, my weight is around 176 pounds, and my height is 5 feet 8 inches. I feel cold more than normal, with body stiffness and headache in cold air since three years. Even with a fan, I feel uncomfortable. My blood tests are normal, and my thyroid is normal, but this issue troubles me a lot.
Kindly help.
Thank you.
Hello,
Welcome to icliniq.com.
I read your query and understood your concern.
From what you have described and the test results that you have attached (attachments removed to protect the patient's identity), it appears that your blood and chemical profile are largely within the normal physiological range.
Normal findings are:
1. Thyroid hormones (based on your report): Normal.
2. Complete blood count: No anemia; hemoglobin excellent (15.1 grams per deciliter).
3. Blood sugar: Fasting 87.5 milligrams per deciliter and HbA1c (glycated hemoglobin) 5.7 percent: good glycemic control.
4. Kidney and liver enzymes: Normal.
5. Creatinine: Normal with good estimated GFR (glomerular filtration rate).
6. Calcium, sodium, phosphorus: Normal.
Mild abnormalities requiring attention:
Vitamin D insufficiency. Level = 29.86 nanogram per milliliter (Normal ≥ 30 nanogram per milliliter). Low vitamin D causes are as follows:
1. Muscular stiffness.
2. Heightened pain perception.
3. Joint aches.
4. Low immunity.
5. Worsened cold sensitivity.
Lipid profile.
Total cholesterol = 221 milligrams per deciliter, which is borderline high.
LDL (low-density lipoprotein) = 153 milligrams per deciliter is high.
LDL and HDL (high-density lipoprotein) ratio = 3.36 is mildly increased cardiovascular risk.
In older adults, high LDL contributes to:
1. Decreased peripheral blood flow.
2. Cold hands/feet and poor heat regulation.
3. Headache due to vascular reactivity.
Eosinophils are slightly high. Absolute eosinophil count ~0.51 (upper limit ~0.5). Suggests allergic tendency, sinus hypersensitivity, or environmental trigger. Cold air can worsen these symptoms, including headache and nasal congestion.
Bilirubin is slightly elevated. Total bilirubin = 1.45 milligrams per deciliter. Indirect bilirubin = 1.25 milligrams per deciliter. Can cause:
1. Tiredness.
2. Increased sensitivity to stress and cold.
The likely explanation of your symptoms is as follows:
Even with a normal thyroid, cold intolerance has several other physiological causes. In your case, the most probable explanation is multifactorial:
Vitamin D insufficiency: Muscular stiffness, heightened cold sensitivity. Vitamin D deficiency reduces muscle efficiency and increases neuromuscular pain when exposed to cold.
Peripheral vasoconstriction tendency due to age and lipids. With advancing age and higher LDL cholesterol, blood vessels lose flexibility. In cold weather, the blood vessels constrict more, and a cold body causes a headache.
Cold-triggered headaches or sinus-related neural sensitivity. Cold air can stimulate trigeminal nerve endings in the face and head. The frontal sinus mucosa causes pain and headache in cold. This explains your discomfort with even a fan or air conditioning airflow.
Mild allergic component (eosinophilia). Cold-induced rhinitis causes cold headache, sinus pressure, and a stiff sensation around the head.
Conditions to consider (clinical differential diagnosis) are as follows:
1. Cold-triggered migraines or sinus headaches cause pain with a fan and breeze.
2. Vitamin D-related myopathy causes muscle and joint stiffness.
3. Mild dysautonomia causes a heightened response to cold.
4. Early osteoarthritis causes age-related stiffness.
None of these is dangerous, but they cause chronic discomfort.
I suggest this treatment plan:
The focus is on correctable physiological contributors.
Vitamin D repletion: Start vitamin D3 50,000 IU once weekly for 6 weeks. Benefits expected in six to ten weeks:
1. Reduced stiffness.
2. Better muscle strength.
3. Less cold sensitivity.
Recheck vitamin D after three months.
Cholesterol and circulation improvement. Lifestyle actions:
1. 30 minutes brisk walk daily.
2. Warm-up stretches before going out in the cold.
3. Foods to reduce LDL: Oats, flax seeds, walnuts.
4. Green leafy vegetables.
5. Garlic.
6. Avoid deep-fried foods, butter, excessive paneer, and red meats.
7. If LDL remains >130 milligrams per deciliter after three months of lifestyle therapy, consider statin therapy.
Protection from cold air stimuli:
1. Avoid fan or air conditioning directly on the face.
2. Use light woolen clothing on the neck and ears.
3. Consider a face mask outdoors in cold wind.
4. Warm showers in the morning improve circulation.
This reduces headache episodes and muscle tightness.
Home strategies for body stiffness:
1. Perform daily: Neck mobility exercises, shoulder rolls, back stretching (ten minutes), and gentle yoga if comfortable.
2. Heat therapy: Warm towel to neck and shoulders 10 to 15 minutes per day.
3. Omega-3 supplements may also reduce neuromuscular stiffness: Fish oil capsules (1000 milligrams per day) or a vegetarian omega-3 source.
Headache management:
1. Occasional medications: Paracetamol or Naproxen.
2. If headaches occur more than eight to ten times per month, a neurology review may be needed for migraine prophylaxis.
3. Steam inhalation once daily for seven days reduces sinus sensitivity.
I hope that this answers your query.
Kindly follow up if you have more doubts.
Thank you.
Patient's Query
Hello doctor,
Thank you for the reply.
Can I have hidden hypothyroidism, and should I take Thyroxin 25 milligrams or 50 milligrams for some days?
Kindly help.
Thank you.

Welcome back to icliniq.com.
I read your query and understood your concern.
I do not think that you have any thyroid problems; therefore no indication to take thyroid medication.
I hope that this answers your query.
