Can low Hb cause a frequent cough in infants?

Hello doctor,

My baby is 11 months old. He has been experiencing cough and cold symptoms for the past month, although not continuously. Sometimes he is well for a few days, and then the symptoms return.

His body weight is 8.5 kg (18.7 pounds), and his hemoglobin level is around 8 g/dL.

His CBC and CRP reports are normal.

The doctor recommended the following syrups:

  1. LS syrup.

  2. Omnacortil syrup.

As a pediatrician (a child specialist), I can understand your concern. It is reassuring that the CBC (complete blood count) and CRP (C-reactive protein) do not suggest a significant bacterial infection.

Recurrent cough and cold symptoms in an 11-month-old child are often due to repeated viral infections, allergies, or reactive airway disease, which may be why your doctor prescribed montelukast, levocetirizine, and a short course of Omnacortil.

However, I am more concerned about the hemoglobin level of around 8 g/dL, which indicates moderate anemia and requires further evaluation. Iron deficiency is the most common cause at this age, but other causes should also be considered.

I would like to know whether your child is breastfed, formula-fed, or taking cow's milk and whether he has started iron-rich complementary foods.

Please provide the following information:

  1. How long has the cough been present?

  2. Is there wheezing, fast breathing, or chest indrawing?

  3. What is the exact hemoglobin value and MCV (mean corpuscular volume), if available?

  4. Has your child been started on iron supplementation?

If the cough persists for more than four weeks, or if there is poor weight gain, recurrent chest infections, wheezing, or breathing difficulty, further evaluation may be needed.

Regarding the anemia, I recommend discussing iron studies and iron therapy with your pediatrician, as a hemoglobin level of 8 g/dL should not be ignored.
