Hello doctor,

I have a lot of dandruff on my scalp. I have visited many dermatologists and used various shampoos to remove my dandruff, but there has been no change. My scalp does not get clean.

Currently, I am using an anti-dandruff shampoo, but there is still no improvement. I still have a lot of dandruff, and there is also hair breakage because I had a hair treatment one year ago. Now there is breakage due to dryness, dandruff, and itchiness. There are many patches of dandruff on my scalp.

The shampoo you are using may help mild flakes, but not thick patches or inflammation, and it does not adequately suppress fungal overgrowth or inflammation.

I suggest you follow this proper treatment that will help you manage it effectively:

  1. Use Ketoconazole 2% shampoo and Ciclopirox olamine shampoo, or Selenium sulfide. Apply the shampoo on a dry scalp, massage gently, leave for five to seven minutes, then rinse. Use only two to three times per week.
  2. Use Betamethasone lotion or Betamethasone and Salicylic acid combination lotion, whichever you can get easily, on your scalp every night like oiling, and leave overnight. Use it for two weeks, then once a week for two weeks as maintenance. This will effectively remove excess dandruff and inflammation.
  3. Avoid hot showers, excessive heat styling, and hair oiling for a while.

Continue this regimen, and you should have a clear and smooth scalp in two to three weeks.
