Hello doctor,

Lately, I have been very tired and have not wanted to do anything. I have been coming home from work and getting into bed, yet still having a hard time waking up in the morning. Physically, my body just feels off, like every movement is harder than it should be. Also, I am having a hard time concentrating. I have noticed it within the past two weeks. I am currently taking Lexapro for anxiety. Please help.

I thoroughly read your query and understand your concern.

The differential diagnosis is MDD (major depressive disorder). I suggest you do a TFT (thyroid function test). Kindly do a self-rating test of depression like Zung's self-rating depression scale, which you can easily find online, and if it is indicative of moderate to severe depression, consult a psychiatrist in person for detailed evaluation and treatment. You can discuss with your treating psychiatrist regarding increasing the dose of Lexapro (Escitalopram) or adding Modafinil. Also, I suggest you follow the following steps:

  1. Sleep hygiene, like sleeping and waking up at the regular time.
  2. Avoid caffeinated drinks at least eight hours before sleeping.
  3. Avoid television, mobile, laptop, etc, at least half an hour before sleep.
  4. Practice relaxation techniques like breath relaxation exercise daily.
