How do I manage my increasing urination difficulties?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have had increasing urination difficulty for 18 months, including weak stream, straining to start, incomplete emptying, increased frequency, and waking 7 to 8 times nightly, which exhausts me. My prostate-specific antigen (PSA) was 14.6 nanograms per milliliter.

Kindly help.

Hello,

Welcome to icliniq.com.

I understand your concern.

Your difficulty starting urination, a weak stream, straining, incomplete emptying, and frequent nighttime urination can be very distressing and exhausting, especially when it has been going on for so long. However, how much have these urinary symptoms been affecting your daily life and sleep? Given your age and the elevated prostate-specific antigen (PSA) level of 14.6 nanograms per milliliter, these symptoms may be related to an enlarged prostate, known as benign prostatic hyperplasia (BPH), or in some cases, prostate inflammation (prostatitis) or even prostate cancer. Prostate-specific antigen (PSA) can rise for several reasons, so it is important not to jump to conclusions, but it does warrant further evaluation.

Your doctor will likely recommend a full urologic assessment, which may include a digital rectal exam (DRE), prostate imaging such as ultrasound or magnetic resonance imaging (MRI), and possibly a prostate biopsy if cancer needs to be ruled out. Even if it turns out to be benign enlargement, there are several effective treatments available, ranging from medications that relax the prostate and bladder muscles to therapies that shrink the prostate or improve urine flow. In more advanced cases, minimally invasive procedures or surgery can offer lasting relief.

In the meantime, try to stay well hydrated during the day but limit fluids a few hours before bedtime to reduce nighttime trips. Avoid caffeine and alcohol, as they can worsen urinary urgency. Most importantly, please see your urologist soon for a detailed evaluation. With proper diagnosis and management, you can significantly improve your quality of life and rest more comfortably at night.

I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.

Thank you.
