Hi doctor,

I am a 30-year-old female. My last menstrual period started on the 19th of last month, and today is the 25th. I did not get my period, so I took a pregnancy test and it came out positive. I already have a medical termination kit with me, but I am unsure if it is safe to use. I am not ready for a third baby, as I gave birth only 6 months ago. At the moment, it is not possible for me to visit a clinic for a surgical abortion.

Since your urine pregnancy test is positive just a few days after a missed period, the first important step is to get an ultrasound scan. This will confirm the location of the pregnancy and how far along you are, while also ruling out ectopic pregnancy or an early miscarriage.

Because you delivered six months ago and are breastfeeding, you need to be especially careful. The medicines used for termination (like Mifepristone and Misoprostol) can pass into breast milk in small amounts, sometimes causing loose stools, irritability, or drowsiness in the baby. Timing feeds or briefly discarding milk may be necessary if treatment is done.

Please do not start the medical termination kit on your own. The tablets must be taken step by step under gynecologist supervision. Usually one dose first, followed by another set after 24–48 hours. The correct dosage and monitoring are very important. Without this, there is a risk of heavy bleeding, incomplete abortion, or infection.

It would also be advisable to:

  • Check your hemoglobin to ensure you are not anemic.
  • Know your blood group, since this matters in case of heavy bleeding or if you are Rh-negative and may need an injection.
  • Keep someone with you during the process and ensure you have quick access to a hospital if you develop severe pain, heavy bleeding (more than two pads per hour), fainting, or high fever.

Once this situation is managed, contraception is important since fertility can return quickly, even during breastfeeding. You may consider options like copper-T, progestin-only pills, or condoms.

After the procedure, you will also need a follow-up ultrasound after 10–14 days to make sure the uterus is completely cleared. Please share your scan findings and blood group report before you proceed, and I strongly recommend either a teleconsult or a local clinic visit before starting the kit.
