Hi doctor,

My period is delayed this month. I usually have a regular 28-day cycle. I have been experiencing period-like cramps and other symptoms, but no bleeding yet. I took a pregnancy test recently, and it was negative. I do not have any known health issues like thyroid problems, PCOD, or PCOS. This is the first time I have experienced a delayed period. What could be the possible reasons?

A single episode of mild fever, cold, stress, travel, sudden weight change, or even a random hormonal fluctuation can delay ovulation slightly, which then causes a shift in the period. This is quite common and nothing to be alarmed about when it happens once in an otherwise healthy and regular cycle.

Investigations to be done:

  1. If still no period after ten days, repeat the urine pregnancy test.

  2. If the delay persists for more than two weeks, do a serum beta-human chorionic gonadotropin (beta hCG) test.

  3. If periods do not come for more than six weeks, a basic hormonal profile, including thyroid-stimulating hormone (TSH), prolactin, follicle-stimulating hormone (FSH), luteinizing hormone (LH), and a pelvic ultrasound, may be advised.

Right now, no specific treatment is needed. Wait for another week; your periods may start on their own. Focus on hydration, balanced nutrition, and adequate rest. If your cycles continue to be irregular in the future, further evaluation may be necessary.

If your period does not start within 10 to 14 days, repeat the pregnancy test and update me. If your period is missed for more than six weeks or you develop new symptoms like galactorrhea (milky nipple discharge), abnormal weight gain, or excessive hair fall, follow up with the required test reports for further review.
