Hello doctor,

I cannot tell if I am experiencing osteonecrosis of the jaw or if a bone spur is coming through my top gum. I have never been treated for cancer or with the medications that would cause it. I have never experienced any trauma on my top teeth that would cause a bone spur to pop up. But it began as a spot on my gum that felt like a hard pimple sized area and then it became bothered and eventually a bone has shown through. I am not sure if it is a spur that needs to be returned or I have osteonecrosis of the jaw?

I went through your query and attached image (attachment removed to protect patient identity).

Kindly upload an image showing the chewing surface of the teeth on the upper left jaw. I want to see the health of the tooth associated with the lesion you have marked.

Nothing to worry about as I am suspecting chronic abscess might lead to bone loss and opening like that. It is not related to the third molar extraction. I would also like to see the bottom third molar extraction site. If that area is healed well you have nothing to worry at all. Just upload me both the images (use the flash) chewing surface of teeth of the upper left jaw and lower third molar extraction site. And if you have any x-ray kindly upload them too.

I would recommend capsule Sporidex 500 mg (Cephalexin 500 mg) one capsule BD for five days, tablet Metronidazole 400 mg BD for five days, tablet Ibuprofen sos (if pains), capsule Omeprazole BD before meals for five days.

Use Betadine (Povidone iodine 2%) mouthwash, gargle in lukewarm water twice or thrice a day. Apply Oraways ointment (Triamcinolone acetonide buccal paste) or Metrogyl DG gel or Rexidine M Forte gel anyone containing (Chlorhexidine + Metronidazole+ Lignocaine). Avoid sticky or sweet food as much possible, rinse whenever you eat.

Start the medications. Upload the images along with if any x-ray present with you. Until then, do not worry much. Neither osteonecrosis nor any other serious issue. Serious abscess might cause these issue.

Suspected chronic abscess.

Investigations to be done

Upload the images along with if any x-ray present. If lower wisdom tooth x-ray present that will also do.

Treatment plan

Start the medications for now. Infection and abscess can cause these issues. Rest , when I recieve the images I will assist immediately.

Preventive measures

Avoid tooth brush rubbing over that area . Apply ointment on the exposed bone and it will heal eventually.

Same symptoms don't mean you have the same problem. Consult a doctor now!
