How to treat depression effectively?
Patient's Query
Hello doctor,
I was on Lexapro 20mg for 3 years and decided to come off it because I was feeling good and like myself again. A year later, I crashed hard, and my depression came back. I recently went back on Lexapro in May, and it helped a bit, but not as it used to, I started gaining weight even though I was eating healthy and exercising regularly. I did not notice this the first time I took it because I was not watching my diet or exercising. My doctor switched me to Pristiq (a direct switch from Lexapro 20mg). I have been on 50mg for nearly 5 weeks, and I do not feel any better, if anything, I feel worse. He said it should be working by now and suggested doubling my dose, but I am not sure if I should give it more time. I feel like I might be having withdrawals from Lexapro since I switched immediately without reducing the dose. A second opinion would be appreciated.
Please help.
Thank you.
Hello,
Welcome to icliniq.com.
I understand your concern.
-
Yes, the sudden switch from 20 mg is not advisable.
-
So you need to reduce dosage gradually.
-
But rather I would suggest you need to continue Lexapro 20 mg and then add another antidepressant or mood stabilizer for better improvement.
-
Lithium Or Bupropion are good options.
-
With pharmacotherapy start non-pharmacological treatment like exercise and psychotherapy.
-
Maintain a healthy lifestyle.
-
Avoid smoking and drinking if any.
-
Do exercise daily at least 45 min a day.
I hope this helps.
Kindly follow up if you have more concerns.
Thank you.
