How to regain penile sensation after excessive masturbation?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have been experiencing a loss of sensation in my penis, which I believe may be due to masturbation over the past nine years. For about a year now, I have had difficulty getting and maintaining erections, and I no longer experience morning erections. I can only achieve an erection after prolonged stimulation, such as rubbing the penis. Could this be a sign of nerve damage? How can I improve sensation in the penis and achieve firm erections?

Please help.

Hello,

Welcome to icliniq.com.

Thank you for sharing your concerns. What you are experiencing is more common than you might think, and it is good that you are seeking help early. Let me walk you through this:

What could be causing your symptoms?

Based on what you have shared, loss of penile sensation, weaker erections, and no morning erections, a few possible causes are:

  1. Desensitization due to overstimulation: Long-term frequent masturbation, especially with a tight grip or pressure (against hard surfaces or using excessive friction), can lead to temporary reduced sensitivity of the penile nerves. This is sometimes called “death grip syndrome” in informal terms. It is not permanent but can take time to reverse.
  2. Mild nerve fatigue or pelvic floor tension: Constant clenching of the pelvic muscles (during masturbation or otherwise) can cause muscle imbalance and neural irritation, sometimes mistaken as nerve “damage”.
  3. Psychological factors (performance anxiety): Worrying about erection quality can, paradoxically, make the problem worse. This is called psychogenic ED (erectile dysfunction) and is very common in younger men.
  4. Hormonal imbalance or lifestyle issues: Factors like low testosterone, poor sleep, stress, obesity, or a sedentary lifestyle can also reduce morning erections and sexual performance.

What tests would help?

To get a better understanding, I would suggest starting with the following:

  1. Serum testosterone (total free).
  2. Fasting blood sugar or HbA1c (glycated hemoglobin).
  3. Thyroid function tests.
  4. Basic lipid profile.
  5. Vitamin D levels.

What can you do to improve this?

Here are some practical steps to begin your recovery:

  1. Stop or reduce masturbation for three to four weeks. Let your nerves reset. Avoid porn and “death grip” masturbation. This is often the single most helpful step.
  2. Start gentle pelvic floor relaxation exercises. These include deep breathing, yoga, and avoiding constant clenching. Overactive pelvic muscles can compress penile nerves.
  3. Improve sleep, diet, and exercise. Aim for seven to eight hours of sleep, daily walking, and a protein-rich diet. Avoid alcohol, smoking, and high-sugar foods.
  4. Try a daily natural erection schedule. Every morning, try to stimulate a soft erection using only mental stimulation and minimal physical touch. This helps restore neurovascular feedback.
  5. If needed, we can consider a short course of medications. Medicines like Tadalafil 5 mg daily can help improve blood flow and erection quality during recovery. Consult your specialist doctor, discuss with him or her, and take the medicines with their consent.
  6. Check testosterone levels. If your testosterone is low, that may need to be addressed, either naturally or with medication.

Is this nerve damage?

Not likely permanent. If you still feel some sensation and can get an erection through touch, it suggests functional nerve pathways are intact. Most cases like yours are reversible with time, habit change, and occasionally medication.

Please do not hesitate to reach out again once your test results are available.

Thank you.

Patient's Query

Hello doctor,

Thank you for the reply.

My serum free testosterone level is 350 ng/dL. The rest of my test results are within the normal range. I have been taking vitamin D supplements for the past two weeks. Even if there is some degree of nerve damage, is it possible for this to be reversed so I can regain full functionality? Or is there a chance that I will be stuck with the current level of reduced sensation permanently?

Please help.

Hello,

Welcome back to icliniq.com.

Thanks for getting back with your results. Your serum testosterone (350 ng/dL) is on the lower end of normal, not alarmingly low, but in some men, especially younger ones, it may still contribute to weaker erections, fatigue, or reduced libido. Good that you have already started vitamin D supplementation, which can support testosterone production and overall mood and energy.

Regarding your main concern, is it reversible?

The short answer is: Yes, recovery is very much possible. Let me explain in a bit more detail:

  1. Nerve desensitization is not the same as permanent damage: What you are experiencing is most likely functional desensitization, not true nerve destruction. Think of it like “overused” sensory circuits that need rest and recalibration. This can absolutely improve.
  2. The nervous system can regenerate: Peripheral nerves, including those in the penis, have excellent regenerative capacity, especially in younger individuals like yourself. With proper care, rest, and circulation, pelvic health function generally returns over time.
  3. Improvement timeline: Recovery may take a few weeks to a few months, depending on how long the overstimulation has been happening. But most men report significant improvement by the eight to 12 week mark after making the right changes.

What you should continue doing:

  1. Avoid rough or frequent masturbation completely for now. Give your nerves rest. Even erections can be allowed naturally, but avoid stimulating them deliberately for a while.
  2. Vitamin D supplementation: Continue as advised, usually for six to eight weeks or until levels normalize.
  3. Consider Tadalafil 5 mg daily for four to six weeks. It is safe, improves blood flow, and supports recovery even if erections are not your immediate goal. Consult your specialist doctor, discuss with him or her, and take the medicines with their consent.
  4. Kegel relaxation, not tightening, and avoid over-engaging the pelvic muscles. You might even try reverse kegels or guided pelvic floor relaxation videos.
  5. Reduce screen time, porn exposure, and stressors that may be overstimulating your brain and dopamine circuits.
  6. Stay active, regular physical activity (walking, weights, yoga) helps both vascular and nerve function.

Based on what you have shared and your test results, there is no sign of permanent damage. Everything suggests reversible, functional suppression, which can recover fully with time, care, and good lifestyle support. Stay consistent and patient. I am confident you will notice improvement.

If things do not begin improving by the eight to 10 week mark, we can consider further tests like penile Doppler ultrasound, but I do not think they will be necessary in your case right now. You are on the right path.

Warm regards.
