Hello doctor,
My 29-year-old wife has had type 1 diabetes since her teenage years, and her condition has generally been stable. However, over the past few weeks, she has been feeling unusually fatigued and experiencing episodes of a rapid heart rate, especially after meals. Her recent hemoglobinA1c level was 7.8 percent, which her endocrinologist mentioned is slightly higher than ideal. She uses an insulin pump, but her glucose levels have been fluctuating, with highs after meals and sudden drops overnight. Her recent blood tests also showed signs of anemia, which I understand could make managing diabetes more challenging. Could the anemia be contributing to her current symptoms? Should we see a hematologist to investigate further, or is this something her endocrinologist can manage? I am also wondering if adjusting her insulin therapy might help stabilize her blood sugar and reduce these fluctuations.

In people with anemia, glucose test results can show lower values. This means that higher blood sugar levels might not be recorded accurately because of the anemia.
Anemia can also cause heart-related issues, which can lead to symptoms like the ones you described.
If your wife has had any recent viral, urinary tract, or respiratory infections and has been treated for them, she might still be in the recovery phase, which could be contributing to these symptoms.
It would be a good idea to consult a cardiologist, who can perform an ECG (electrocardiogram) and an echocardiogram and possibly recommend a blood transfusion if necessary.
Please do not adjust her insulin therapy on your own, as we still do not know the underlying cause of these symptoms.
