How does diabetes impact wound healing?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am a 54-year-old male, who has been on Metformin medication for the past seven years. Recently, I injured my right foot, specifically the first toe. It has been two weeks, and it still has not healed. Could the slow healing be related to high blood sugar levels? Is there a connection between diabetes and the healing process? I have never missed my medications, so I am concerned about why my blood sugar levels are not controlled. My fasting blood sugar levels typically range from 200 to 240 mg/dl. How can I better control my blood sugar levels? Would reducing my blood sugar levels help with the healing process?

Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

I understand your concern.

I am sorry to hear about the health problems you are experiencing. It is concerning that your injury has not healed after two weeks, and your high blood sugar levels could indeed be playing a significant role in this delayed healing.

When blood sugar levels are elevated, it can impair blood circulation, particularly in extremities like your feet. This reduced blood flow means that less oxygen and nutrients reach the injured area, slowing down the healing process.

High blood sugar can also weaken your immune system, making it harder for your body to fight off infections and repair damaged tissues. Additionally, diabetes can cause nerve damage (neuropathy), which may prevent you from feeling pain in the affected area, potentially leading to unnoticed injuries or infections.

1. Why is your blood sugar level uncontrolled?

Your diet plays a crucial role in managing blood sugar. A high intake of carbohydrates, particularly refined sugars, can spike blood sugar levels. Lack of regular physical activity can also contribute to higher blood sugar levels. Sometimes, Metformin alone may not be sufficient to control blood sugar, especially if your body's insulin sensitivity has changed over time.

Lowering your blood sugar levels can significantly improve the healing process. As your blood sugar becomes better controlled, your circulation improves, and your immune system functions more effectively, which can aid in the healing of your injured toe.

Steps to control blood sugar levels:

Dietary Adjustments:

  1. Focus on a low-carbohydrate, high-fiber diet. Include more vegetables, lean proteins, and whole grains.

  2. Avoid sugary snacks, sodas, and processed foods.

  3. Consider working with a dietitian to develop a meal plan tailored to your needs.

Regular Exercise:

  1. Aim for at least 150 minutes of moderate-intensity exercise per week, such as brisk walking or swimming.

  2. Exercise helps improve insulin sensitivity and can lower blood sugar levels.

Medication Review:

  1. Discuss with your healthcare provider whether your current dose of Metformin is sufficient or if additional medications might be needed to better control your blood sugar levels.

  2. Monitoring: Regularly monitor your blood sugar levels at home. Keeping track of these readings can help you and your doctor adjust your treatment plan if necessary.

I hope that you get your answer.

Please, let me know if you want some help.

Thank you.
