Hello doctor,
I am a 28-year-old woman with type 1 diabetes, and I was recently diagnosed with PCOS (polycystic ovarian syndrome). My insulin needs have become unpredictable, and I am currently using Lantus (32 units daily) and Humalog (six to twelve units per meal, depending on the time). My latest lab results show a hemoglobin A1c of 8.4%, elevated testosterone levels at 68 milligrams per deciliter, fasting glucose of 185 milligrams per deciliter, and irregular menstrual cycles. I am using a Dexcom G6 CGM (continuous glucose meter), which shows a lot of glucose variability between 65 to 290 milligrams per deciliter. My doctor recommended adding Metformin to my insulin therapy to help with insulin resistance. I have also developed acne and other skin issues. I am especially worried about how these conditions might affect my fertility and how to manage both diabetes and PCOS. What do you recommend for managing these complex health issues?
