Hello doctor,
My father is 84 years old and has been diabetic for the past 35 years. He had a bypass in the past and also a stroke. He also underwent treatment for non-Hodgkins lymphoma a few years ago. He is taking the following medicines:
Mixtard Insulin 30/70 - 55 units in the morning and 12 units at night.
Glycomet GP2 morning and night.
Linagliptin 5 mg every morning.
Fibator 10 mg every night.
Telma CT 12.5 half tablet in the morning.
Dynapress 0.4 at night.
Arreno morning and night.
Ecosprin 75 in the morning.
Thyronorm 25 mg, one tablet every Monday and Thursday and two tablets Friday to Sunday.
Amlong 2.5 at night.
Capsule Uprise once a month.
Rejuron OD - one tablet five times a week.
Maxgalin 75 mg at night.
Please examine his routine blood test reports. He is fine, apart from his right side pain and discomfort, which he has been having since his stroke 22 years ago. Some parameters in his thyroid test and LFT and KFT are slightly out of range. Hope they are fine for his age. Also, his platelets have been in the range of 1.2 lakh to 1.5 lakh in the last 15 years.

According to your statement, your father has diabetes, a history of stroke, and a history of non-Hodgkin's lymphoma. The test report results (attachments removed to protect the patient's identity) suggest anemia, hyponatremia, low sodium, thrombocytopenia, or low platelet count. There are some changes in his blood test results, but those changes may be within normal limits according to his age. His prolonged thrombocytopenia, or low platelet count, may have resulted from drug induction. Your father can take iron supplements or iron-enriched foods with citrus fruits that will help increase iron levels and may help remove his weakness. Taking oral rehydration salt can help to correct his sodium levels, or hyponatremia, and remove his current weakness if his blood remains within normal limits. Taking these measures may help to improve his current health condition.

Thank you for your reply. How long does he need iron supplements, and is Orofer XT good? Also, could you see his BUN test readings and his microalbuminuria? Kindly suggest.

His sodium level is within normal limits, so there is no hyponatremia, and an oral rehydration solution should not be needed. Yes, your dad can take one capsule of Orofer XT (Ferrous ascorbate and Folic acid), which is good and is widely used as an iron supplement. He can take one capsule for two to three months and then repeat his CBC (complete blood count) report. Again, microalbuminuria can be increased with age and increased duration of diabetes mellitus. Similarly, BUN (blood urea nitrogen) readings can also be increased with age. So, according to his age, his BUN reading and microalbuminuria evidence are nothing to worry about.
