Hello doctor,

I had symptoms of diarrhea for almost three weeks immediately after eating. I also feel nauseous with large meals, so I only eat broth and bread. I do not feel anything while drinking water. I took a test that showed no bacteria, parasites, or ova. Also, I saw blood in my saliva last night while brushing my teeth. Kindly help.

I read your query and can understand your concern.

From your history, it seems to be either persistent diarrhea with an infective agent, which is not localized on your reports (attachments removed to protect the patient's identity), or another possibility is post-infectious irritable bowel syndrome. This happens after a short episode of infection. Later on, the infection will be gone, but gut sensitivity persists for a few weeks. After eating, the urge to go to the toilet is normal gastrocolic reflux, present in everyone, but with the above two possibilities, this increase significantly like yours.

I suggest the following.

  1. Take probiotics for two weeks to replace harmful Bacteria with good healthy bacteria.
  2. Avoid eating out or reheating meals.
  3. Try to have fresh home-cooked meals.
  4. Avoid commercially available juices, tea, or coffee, as they increase diarrhea.
  5. Take anti-spasmodic medications like tablet Mebeverine 135 mg or tablet Drotaverine.
  6. Nausea will improve by eating frequently but a small amount. So do not fill your stomach for a few days.
  7. Take antacid syrup like Gaviscon or simple acid suppression medicine like tablet Pantoprazole 40 mg once daily half an hour before meals.
  8. Regarding bleeding, use a soft-bristled toothbrush while brushing because I suppose it is not much related to your gut symptoms, specifically if you have not noticed anything red in your vomiting.

Hello doctor,

Thank you for the reply.

Before this all started, I had intense heartburn and took the Pantoprazole tablet, which helped immediately. My doctor also prescribed me Diphenoxylate for persistent diarrhea. She also asked me to take Pantoprazole.

As far as the blood, I have seen this a few times. I know it is not from gum bleeds. It is from the throat.

Can I take both tablets, and what is the cause for the bleeding?

You can take tablets Pantoprazole (proton pump inhibitor) and Diphenoxylate (anti-diarrheal) with you during the trip. Also, try to eat hygienic food and plenty of water.

Regarding your occasional bleed, if it comes from the throat, consult a specialist to check if you have a sore throat or allergic pharyngitis.
