I was scheduled for a hysteroscopy with D and C. Is it safe?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I had a miscarriage at six weeks. I was scheduled for a hysteroscopy with D&C to remove retained product conceptions. The operating surgeon came and spoke about all complications that can happen, like bladder perforation, uterus perforation, incision due to any perforation, uterus removal due to any complication, uterus scarring, etc.

Can this surgery cause so many complications?

Kindly help.

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

Hysteroscopy combined with dilation and curettage (D and C) is a standard and generally safe procedure used to remove retained products of conception after miscarriage, especially when ultrasound confirms their presence.

While the surgeon appropriately counseled you about potential complications such as bladder or uterine perforation, intraoperative injury, and uterine scarring (Asherman’s syndrome), which can happen in rare cases, it is essential to understand that these are uncommon and represent worst-case scenarios rather than routine outcomes, so do not worry.

Perforation occurs in a small percentage of cases, usually without lasting damage, and most procedures proceed uneventfully. Proper surgical technique, experienced operators, and intraoperative visualization via hysteroscopy significantly reduce these risks. Uterine scarring is a concern if multiple procedures are needed or if infection develops, but it can often be minimized with careful follow-up.

Overall, the benefits of removing retained tissue to prevent infection, heavy bleeding, or future fertility issues usually outweigh these rare risks. If you have concerns, discussing them openly with your surgeon to clarify the likelihood of complications and the postoperative care plan can help provide reassurance and prepare you for recovery.

I hope this helps you.

Kindly follow up if you have more doubts.

Thank you and take care.
