Can a pregnancy test be negative despite missed periods?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

After egg ovulation, I used Folic acid, Dydrogestrone, and micronized progesterone soft gelatin 200 mg for 14 days. Then, I missed my period, but the pregnancy test was negative. I want to know whether I am still pregnant or not. Please suggest.

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

If you missed your period but have a negative pregnancy test, there are a few possible explanations. Pregnancy is still possible. If you tested too soon, your hCG (human chorionic gonadotropin) levels might not be high enough to detect. Try testing again in two to three days using first-morning urine (when hCG is most concentrated). If still negative and no period, consider a blood hCG test for more accurate results. Progesterone delays your period. Both Dydrogesterone and Micronized progesterone can delay your period by keeping your uterine lining thick. Some women do not get their period until a few days after stopping progesterone. The cause can be an anovulatory or irregular cycle. Sometimes, cycles can be delayed due to stress, hormonal changes, or an anovulatory cycle (when no egg is released).

Repeat a urine pregnancy test in two to three days (first morning urine). If still negative and no period, consider a blood hCG test. If your period does not start within a week, consult your doctor about stopping progesterone to trigger a withdrawal bleed. Do you have any other symptoms like breast tenderness, nausea, or cramping?

Let me know if I need to assist you further.

Thank you and take care.
