Hello doctor,
I am so worried because of an infection on my penis. I had sex 25 days before, and I took one tablet of Sildenafil 100 mg before sex. I did not use a condom. In the morning, I have a wound on my penis which becomes bigger day by day. It is like an infection. Now I am worried. Tell me what it is. Are these symptoms of HIV? I do not have any other problem in my body. But at times, it is painful when I urinate.

I have opened all the files you sent (attachments are removed to protect the patient’s identity). It is a very big sore ulcer, and it can be painful. It might be an STD (sexually transmitted disease), consult a skin venereal disease specialist for a test for STD. If in tests your diagnosis is confirmed, then it is so easy to treat. I do not think it is due to HIV (human immunodeficiency virus). It looks like STD may be chlamydia, gonorrhea, etc.
The venereologist may take scraping and then examine the culture and do a sensitivity test, for which a drug can be given. If you are worried about HIV infection, you may go for confirmatory tests like a fourth-generation CMIA (chemiluminescent magnetic microparticle immunoassay) kit test for HIV. Its results are authentic and sure. Till then, I suggest you use Doxycycline 100 mg for seven days and a tablet of Fluconazole 15 mg once per day for five days. Clean the ulcer with warm water and apply an antiseptic solution gently.
