Hello doctor,
I am a 29-year-old male, 5 feet 8 inches tall, and weigh 158 lbs. My problem is not only rash but also frequent skin infections along with eczema. Whenever eczema flares on my arms, after a few days, a yellow crust forms, and the doctor says it is a bacterial infection and prescribes antibiotics. Why does this infection keep happening again and again?
Currently, I am using a moisturizer multiple times daily and Betamethasone cream during flare-ups.
Do I need stronger treatment, such as immunosuppressant tablets?
Also, can sweating during gym workouts worsen eczema?
I notice that the itching increases after exercise. Kindly explain what precautions I should take daily to avoid complications.

Recurrent skin infection over eczema is a relatively common problem. In atopic dermatitis, the skin barrier becomes weak and dry, which allows bacteria to easily enter and multiply on the skin surface. The most common organism responsible is Staphylococcus aureus. Because the protective barrier of the skin is impaired, even minor scratching or friction can lead to secondary bacterial infection, which typically appears as yellow crusting, oozing, or worsening redness.
Another important factor is persistent itching and scratching. Repeated scratching damages the already inflamed skin and creates small breaks through which bacteria can enter. In addition, people with eczema often have higher bacterial colonization on their skin compared to individuals without eczema, which explains why infections may recur.
Regarding your current treatment, regular use of a moisturizer multiple times daily is very important, as it helps restore the skin barrier and reduce flare-ups. Topical corticosteroids such as Betamethasone can help control inflammation during flares, but they should be used in the correct strength and duration as advised by your doctor.
You may not necessarily require systemic immunosuppressant tablets at this stage unless:
The eczema is severe or widespread.
Symptoms are not controlled with topical therapy.
There are very frequent flare-ups despite proper skin care.
In patients who develop recurrent infections, dermatologists sometimes recommend additional strategies such as antiseptic washes, short courses of topical antibiotics for localized infection, or other anti-inflammatory treatments, depending on severity.
Sweating during exercise can indeed worsen eczema symptoms in some individuals. Sweat contains salts that can irritate already sensitive skin and trigger itching. However, this does not mean you should avoid exercise entirely. Instead, some precautions may help reduce irritation. I would suggest you take some daily precautions that may help, including:
Apply a thick moisturizer at least two to three times daily, especially immediately after bathing.
Use mild, fragrance-free cleansers instead of harsh soaps.
Avoid very hot showers and use lukewarm water.
After gym workouts, shower promptly and apply moisturizer again.
Wear loose, breathable cotton clothing during exercise to reduce sweat irritation.
Keep your nails short and avoid scratching as much as possible.
Identify and avoid personal triggers such as excessive heat, sweating, stress, or certain detergents.
If infections continue to recur frequently, it is advisable to follow up with your dermatologist for reassessment. In some cases, treatment adjustments or additional preventive measures may be required to control both eczema and recurrent infections.
