My son has eczema. Can coconut oil or herbal creams help?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My son is 10 and has had eczema since he was a baby. Recently, a few spots on his arms started oozing a little fluid and formed yellow crusts. The pediatrician gave Bactroban ointment to apply along with moisturiser.

  1. Does this mean eczema got infected with bacteria?

  2. How can we prevent this from happening again, because he keeps scratching in his sleep?

  3. Also, relatives suggest coconut oil and some herbal creams, saying it helps eczema. Are these safe to try, or can they irritate skin more?

Kindly suggest.

Hello,

Welcome to icliniq.com.

I understand your concern.

Eczema can be especially difficult for children, and it is very distressing for parents to see the skin become sore or infected.

Children who have atopic dermatitis (the most common type of eczema that begins in infancy) often experience dry, itchy patches on the arms, legs, or body, and frequent scratching can damage the skin barrier.

When the skin barrier breaks, bacteria can enter more easily. The symptoms you described, like oozing fluid, yellow crusts, and increased redness around eczema patches, are classic signs that the eczema may have developed a secondary bacterial infection, most commonly caused by Staphylococcus aureus.

The ointment your pediatrician prescribed, Bactroban (which contains Mupirocin), is a topical antibiotic specifically used to treat these types of localized skin infections.

When applied as directed, it usually helps clear the infection within several days while the moisturizer helps repair the skin barrier.

This situation does not necessarily mean the eczema itself has become worse permanently; it usually means the skin was scratched enough to allow bacteria to enter, which is quite common in children with eczema.

The intense itch–scratch cycle, especially during sleep, is one of the biggest reasons infections occur.

To help prevent this in the future, several strategies can make a big difference: keeping your son’s nails short and smooth, using cotton gloves or soft long sleeves at night, applying thick fragrance-free moisturizers multiple times a day, and using prescribed anti-inflammatory creams when eczema flares start.

Some parents also find that applying moisturizer right before bedtime reduces nighttime itching. If itching is severe, doctors sometimes prescribe mild antihistamines at night to help children sleep and scratch less.

Regarding natural remedies like coconut oil or herbal creams, it is understandable that relatives suggest them, because many traditional remedies are popular for eczema.

Coconut oil in its pure, virgin form can sometimes be helpful as a moisturizer because it has mild antibacterial and skin-barrier-supporting properties.

Some children tolerate it well, especially if it is plain and fragrance-free. However, it should be used cautiously and never on skin that is actively infected or oozing, because oils can sometimes trap moisture and bacteria in those areas.

Herbal creams are more unpredictable: many contain fragrances, essential oils, or plant extracts that may irritate sensitive eczema skin or trigger contact dermatitis, which can make the rash worse.

Because children’s eczema-prone skin is very sensitive, dermatologists usually recommend sticking with simple, medically tested moisturizers rather than experimenting with multiple herbal products.

Overall, the symptoms you noticed. oozing patches, yellow crusts, and worsening redness strongly suggest that the eczema areas became secondarily infected, which is why the pediatrician prescribed Bactroban.

With proper treatment and good skin care, these infections usually clear up quickly. The most important long-term step is protecting your son’s skin barrier and reducing scratching, because that is the main way bacteria get into eczema skin.

I hope this helps.

Thank you.
