I have been feeling epilepsy symptoms for the past 20 days. I am feeling dizzy, and there is a change in smell and taste.

Your MRI (magnetic resonance imaging) brain report shows a right temporal lobe intraparenchymal hemorrhage (small bleed) with mild surrounding swelling (15.8 cc) (attachment removed to protect the patient’s identity). This area of the brain is important for smell, taste, memory, and seizure control, which explains your recent symptoms.

The dizziness, altered smell, and taste can be part of temporal lobe seizure activity following such a bleed. The good sign is that your MR angiography (Magnetic Resonance Angiography) and venography are normal, meaning no aneurysm, vessel malformation, or clot is seen. There is mild mass effect but no major pressure or herniation, suggesting a stable, non-surgical lesion at present. Your dizziness and sensory changes may represent focal seizures (aura type) due to local irritation around the hematoma. These can evolve into full seizures if left untreated.

Treatment includes-

  1. Antiepileptic medication to prevent seizures (example - Levetiracetam, Valproate, or Lacosamide, depending on tolerance).

  2. Strict blood pressure and sugar control are required to prevent rebleeding.

  3. Avoid driving, swimming, or working at heights till seizures are fully controlled.

Have a follow-up MRI (magnetic resonance imaging) in four to six weeks to assess hematoma resolution. If you experience any loss of consciousness, jerking, confusion, or worsening headache, seek immediate medical help. With proper medication and monitoring, recovery and seizure control are usually good in young patients like you.
