Patient's Query

Hello doctor,

I have been having fear and anxiety for four weeks. For the first four days, I took Alprax 25 mg. There were no improvements. Then, I took Olanzapine 5 mg for four days but it caused strange behavior. MRI of the brain indicates that everything is normal. Then after changing the doctor, another doctor prescribed Clonazepam 0.25 mg twice daily for eight days. There were a lot of improvements. So, I thought everything is normal. But after one day, the symptoms appeared again. Then he prescribed Fludac 20 mg in the morning and Clonazepam 0.25 mg daily. I am taking the medicines but I have been suffering from insomnia for the past three days. Please help.

Hello,

Welcome to icliniq.com.

I can understand your situation and I empathize with you. Fear and anxiety do not come without any reason. You mentioned that you have been suffering from this issue for the past one month. So, did you face any situations or conditions that provoked your anxiety? Medicines will give you symptomatic relief if the cause is present. For insomnia, you can take the tablet Clonazepam 0.5 mg or 1 mg as required.

I hope this helps.

Thank you.

Patient's Query

Hello doctor,

Thank you for the reply.

I am not feeling well. I am also taking Fludac 20 mg for the past six days in the morning. I am taking Clonazepam 0.5 mg twice daily and Inzofresh (Zolpidem 10 mg) at night for two days.

Hello,

Welcome back to icliniq.com.

You should increase the dosage of Fludac (Fluoxetine) to 40 mg. Are you suffering from insomnia after taking Zolpidem and Clonazepam? If you do not mind may I know the real cause behind your illness?

Thank you.

Patient's Query

Hi doctor,

I am not feeling well and there is no insomnia after taking Zolpidem. My sickness started from last month because of urinary infections. For that I visited the doctor and I was prescribed antibiotics and painkiller like Alkasol syrup along with Alprax 0.25 mg. Urinary problems have vanished, but anxiety and fear persist and then, rest is as mentioned.

Hi,

Welcome back to icliniq.com.

I suggest you take Fludac (Fluoxetine) 40 mg and if you have anxiety problem, you can take Inderal (Propranolol) 20 mg twice a day as it will help relieve the symptoms. It is situational. Everything will be all right. Do not worry.

Thank you.

Patient's Query

Hello doctor,

Thank you for the reply.

Please tell me also about Clonazepam 0.5 mg which I am taking twice daily. For how many days and how many times should I take this medicine, or should I stop? How long should I take Zolpidem? For how many days should I take Inderal 20 mg? Also, for how many days or weeks should I take Fludac (20 mg twice)? Kindly suggest.

Hi,

Welcome back to icliniq.com.

Once you get more relaxation then you can reduce the dosage on your own. Stop Fludac (Fluoxetine) as tapering off. Reduce it to 20 mg and then stop. You can stop Inderal (Propranolol) suddenly but continue it till your exam as it will reduce your anxiety.

For Clonazepam, first, stop the morning dose and then see the effect. If everything is normal, then try to stop the bedtime dose and in the last, regarding Zolpidem, use it as SOS (as and when required). Improve your sleeping habit. After that, I think you can stay away from these medications. Do some physical activities, listen to music, do yoga, and relaxation exercises like deep breathing.

I hope this helps. Do get back if you have any further queries.

Thank you.

Patient's Query

Hi doctor,

My exam is over. I am feeling well but the inherent problems (fear in mind without any reason) persist. I have a fear of going out. And even if I am alone for a few minutes, then again, the fear starts especially when parents are not at home. Please help.

Hello,

Welcome back to icliniq.com.

Glad to have you back.

What kind of fear do you have? Are you afraid of being alone or are you afraid of open spaces or closed space or afraid of that someone can harm you? Our fear is somehow related to our childhood memories. If something bad happens in the past, then it automatically presents as fear in the present. Have you had any bad experience in your life that was traumatic? Without resolving it, it is difficult to remove your fear, but with psychotherapy and proper counseling your condition will improve. If you get anxious, you can continue taking the tablet Propranolol (Inderal) 20 mg as and when required.

I hope this helps.

Thank you.

Patient's Query

Hello doctor,

Now my fear is reducing. But my treating doctor has asked me to continue taking the medicine Fludac 20 mg one tablet in the morning. He said that I am in depression, so he also prescribed Hunchi tablet to be taken one in the evening for 10 days. But, I am not so much depressed. I only feel weak. Should I take Hunchi tablet? Please suggest.

Hello,

Welcome back to icliniq.com.

I feel happy to know that your symptoms are subsiding day by day. The symptoms of depression are low mood, lack of interest, fatigue, crying, worthlessness, lack of confidence etc. What do you mean by Hunchi tablet? If you do not have any symptoms, you can stop taking Fludac (Fluoxetine). If you want to stay away from these symptoms, then you have to indulge in activities that give you pleasure and keeps you busy.

I hope you had a pleasant experience at icliniq.

Thank you.

Patient's Query

Hello doctor,

The salt composition of Hunchi tablet is Escitalopram 10 mg and Clonazepam 0.5 mg.

Hello,

Welcome back to icliniq.com.

If you are taking Fludac (Fluoxetine), then no need to take this one. I had mentioned the symptoms of depression. If you have such symptoms, then continue taking the medicine. If not, you need not take.

I hope this helps.

Thank you.
