How to manage anal pain after bowel movements?
Patient's Query
Hi doctor,
I experience a delayed, sore-like pain near the anal area following a bowel movement. The discomfort lasts for about two to three hours and worsens when I sit. I have been diagnosed with external hemorrhoids, but the pain feels more like a burning soreness.
Kindly help.
Hi,
Welcome to icliniq.com
I understand your concerns.
Based on your symptoms, the most likely cause appears to be external hemorrhoids with irritation or thrombosis. The burning sensation and delayed pain after bowel movements are typical signs of irritated or swollen hemorrhoids. Worsening with sitting is common due to direct pressure on inflamed veins.
If the hemorrhoid is thrombosed (clot inside), pain tends to be more intense and prolonged. Please monitor if you have any of the following symptoms, which may indicate a serious condition:
-
Bright red bleeding that is heavy or frequent.
-
Severe swelling.
-
Hard lump or bluish color (possible thrombosis).
-
Fever or pus (suggests infection or abscess).
-
Rapid worsening of pain.
The treatment plan I will suggest is:
-
Topical relief:
-
Use Hydrocortisone cream, which reduces inflammation.
-
Use Lidocaine gel or ointment to help with pain relief.
-
Witch hazel pads (Tucks) for soothing irritation and inflammation.
-
-
Warm sitz baths: Sit in warm water (not hot) for 15 to 20 minutes after each bowel movement and before bed. It helps reduce swelling and pain.
-
Stool softening: Use a stool softener (like Docusate) or fiber supplement (psyllium) to prevent straining, which worsens hemorrhoids.
-
For pain relievers, use Paracetamol or Ibuprofen to help with pain and inflammation.
-
Avoid prolonged sitting. Use a donut cushion to reduce pressure on the area. Take frequent breaks to stand or walk around.
Please follow up in a week for further evaluation.
I hope this answers your query.
Thank you.
