What are the side effects of using skin-lightening creams?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have been using a whitening cream for some time, but later, it started giving me small rashes. I decided to use another cream, but this one worsened the condition, so I stopped using it. I went to the chemist, and he recommended that I use Xtraderm, but it was not helping because it was only making me lighter. Later, I used Hydrochloric acid, which helped for a moment, but it still caused rashes that turned into black spots.

Kindly guide.

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

It sounds like your skin is reacting negatively to harsh or irritating ingredients in these creams, possibly due to steroid overuse, hydroquinone, or other skin-lightening agents. Here is what might be happening:

Possible causes of your skin reaction:

  1. Contact dermatitis – This causes red, itchy rashes due to strong chemicals in the creams.
  2. Steroid withdrawal (If Xtraderm contained steroids) – Some skin-lightening creams contain hidden steroids that thin the skin. When discontinued, they can cause rashes and dark spots.
  3. Post-inflammatory hyperpigmentation (PIH) – When rashes heal, they often leave dark marks, especially in individuals with darker skin tones.
  4. Overuse of harsh exfoliants (Hydrochloric acid ) – This can burn the skin, worsen irritation, and lead to increased pigmentation.

I advise you to do the following:

Stop using all whitening creams immediately. Your skin barrier is damaged, and continuing to use these products will worsen the condition. Avoid steroid creams unless prescribed by a doctor.

The following to be done to repair your skin barrier:

  1. Use a gentle cleanser – Avoid harsh soaps and opt for mild cleansers like Cetaphil or la roche-posay toleriane.

  2. Moisturize intensely – Look for products with ceramides and niacinamide.

  3. Apply sunscreen daily (SPF 50+) – This helps prevent dark spots from worsening.

The following to be done to treat the rashes and dark spots naturally:

  1. Aloe vera or cica cream – These ingredients help soothe inflammation.
  2. Rosehip oil or vitamin C serum – They assist in gradually fading dark spots.
  3. Avoid harsh scrubs and peels – Allow your skin time to heal before using exfoliants.

Kindly consult a specialist doctor, talk with them, and take medications with their consent.

Thank you.
