Hello doctor,
My son is four years and two months old and weighs 33 pounds. He has been suffering from a fever for the past five days.
Initially, the fever was high (103°F), and Ibuprofen was given. However, on day two, he developed stomach pain, so the doctor changed the medication to Paracetamol syrup 5 mL.
After that, on day three, the peak temperature came down to 101 to 102°F, and the frequency of fever episodes reduced to once every 12 hours. On day four, the peak temperature further decreased to 100 to 101°F, but the frequency increased to about every five hours.
Today is the morning of day five. Apart from the fever episodes, he remains active and playful. His appetite is generally good, although the quantity he eats at each meal has decreased slightly because of altered taste. ORS is being given twice a day, and he is drinking it well.
Is this increase in the frequency of fever episodes normal at this stage, or does he need any change in medication?

It is reassuring that your son remains active and playful between fever episodes, is accepting fluids, and has a reasonably good appetite. These are generally favorable signs.
However, a fever persisting for five days should not be attributed solely to the response to fever medications. While the peak temperature has gradually decreased from 103 degrees Fahrenheit to around 100 to 101 degrees Fahrenheit, the persistence of fever suggests that the underlying illness may still be ongoing.
At this stage, I would not recommend changing from Paracetamol to another antipyretic simply because the fever is recurring more frequently. Paracetamol helps control symptoms but does not treat the underlying cause of the fever.
Since this is now the fifth day of illness, I would advise a review by your pediatrician for a physical examination and consideration of investigations such as:
Complete blood count (CBC).
Other tests based on local epidemiology and clinical findings.
In the meantime, you should:
Continue adequate hydration with water, ORS (oral rehydration solution), and other fluids.
Continue Paracetamol as prescribed for fever-related discomfort.
Encourage rest and monitor his temperature regularly.
Monitor his urine output and activity level.
Please seek urgent medical attention if he develops:
Difficulty breathing.
Persistent vomiting.
Severe abdominal pain.
A rash or any bleeding manifestations.
Excessive drowsiness or lethargy.
Poor oral intake or signs of dehydration.
Also, let me know if:
He has any cough, runny nose, sore throat, diarrhea, or urinary symptoms.
Has he developed any rash?
What is the highest temperature recorded?
Have any blood tests been performed since the fever began?
