Is there a non-surgical remedy for forearm fracture?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My mother is 70 years old and had a right forearm fracture. The plaster was done ten days ago. We got an X-ray done today and wanted to discuss the condition and chances of healing and deformity with you.

Hi,

Welcome to icliniq.com.

Thanks for the query.

Sorry for being late. I have seen the X-rays (attachments removed to protect the patient’s identity). There are two things. First, the bones at the fracture site are not aligned in an acceptable position. Second, the bones are very weak known as osteoporosis (a condition of the bones where the quality or structure of the bones alters or when the loss of bone mass and bone mineral density occurs). Now, we have to treat osteoporosis for sure with no doubt. The treatment of a fracture depends on her pre-injury status. How independent she was before the injury? Is she right-handed? Does she have any other medical history like diabetes, hypertension, and heart disease? Kindly provide the mentioned medical history.

Thank you.

Patient's Query

Hello doctor,

My mother was rather independent pre-injury. She is right-hand dominant. She is not taking any medicines for high blood pressure and high sugar. Both are under control.

Hi,

Welcome back to icliniq.com.

If that is the case, then I feel it should be operated. The alignment is not acceptable to me.

Thanks and regards.

Patient's Query

Hello doctor,

What are the non-surgical options available? Can the bone be realigned under a live X-ray to make sure the chances of natural reattachment are high?

Please explain to me.

Thanks and regards.

Hi,

Welcome back to icliniq.com.

The non-surgical option is the reduction of bone and the application of plaster. Yes, it can be aligned under the c-arm which is live X-rays in the operation theatre. But I feel that will not stay stable and the reduction will be lost. So, in my opinion, it is better to operate.

Thanks and regards.

Patient's Query

Hello doctor,

We went for K-wire insertion and this is the X-ray post-surgery. Is there any improvement in alignment?

Kindly check and clear my doubt.

Thanks and regards.

Hi,

Welcome back to icliniq.com.

That is good you have chosen operative management. The reduction looks fine (attachments removed to protect the patient’s identity) and fixation is also fine. I wish her a speedy recovery.

Thank you.
