Hello doctor,

I suffer from chronic itchiness, especially around the groin area. I have severe inflammation on the testicles and penis. The itchiness spreads to the entire body. I have no sexual partner. Is it a fungal infection or a std (sexually transmitted disease)? I developed this condition from my roommate, who has similar symptoms. Please recommend some medicine for itchiness and inflammation. Currently, I am taking an Itraconazole capsule of 200 mg. Please help.

I have read the query, and I can understand your concern.

I have examined your pictures (attachment removed to protect the patient’s identity), and you have scabies, not fungal infections. Scabies is an intensely itchy skin infestation caused by the mite. The diagnosis of scabies can often be made clinically with a pruritic rash. The itching occurs predominantly at night with characteristic linear burrows over limbs, and other family members can be affected. For treatment, I suggest

  1. On the first day, apply 5 % Permethrin cream at night, leave it for 10 to 12 hours, and then use HH Mite soap (Permethrin 5 %) for bathing. Continue bathing daily with the cream and soap for seven days. Also, take the tablet Ivermectin 12 mg single dose the same day, wash your used clothes thoroughly, and use fresh clothes after a bath the next day.

  2. On the second day, apply Fluticasone 0.05% cream twice daily for five to seven days and Tablet Levocetirizine once daily for the same duration.

Consult a specialist doctor, talk to them, and then take the medication. Get a review after one week.
