After going through the history and images (attachments removed to protect the patient's identity) attached, I suppose it is Tinea cruris (affecting folds and unexposed areas) with Tinea corporis (affecting the trunk and other body parts).
Tinea cruris is a fungal infection most often seen on the folds and private areas where exposure is less. They appear due to the stagnation of sweat and moisture. Private areas, groin, and buttock areas are major skin fold areas that would be affected. Underarms, under the abdomen, and folds of arms and legs, are other areas to be affected. They may also form due to decreased immunity, spread from an infected person, hygiene issues, climate changes, and hormonal changes. They appear recurrently.
Due to external and internal triggers, as mentioned above, Tinea corporis is similar but affects exposed areas on the body instead of folds. They present with itching, scaly lesions, sometimes oozing and foul-smelling secretion, and occasionally pain and redness. Itching is the most common feature, which may lead to discomfort throughout the day, especially when sweating. Apart from the same, there are factors to prevent recurrences of the same. Tinea infections are usually on and off and recur due to multiple factors as listed above, due to which its prevention may be done with the below advice.
I suggest the following.
- Use Ketoconazole soap for bathing twice daily. First, lather the soap with water and apply it to affected areas only. Keep for three minutes and then wash off. Do not use this soap on unaffected areas. Instead, kindly use regular soap on other unaffected areas of the skin.
- Apply Terbinafine cream twice daily. After bathing, clean the areas, towel dry, air the areas to get rid of moisture and apply the cream. Make sure to wash your fingers well after applying the cream. They may be continued for months.
- Apply Terbinafine dusting powder in the afternoon and evening on the affected area to eliminate itching and absorb moisture. They may be continued for months.
- Take capsule Itraconazole 200 mg once daily at night after food for four weeks.
- Take tablet Cetirizine 10 mg one hour before sleep at night for one week. Ensure not to go out or drive after consuming this tablet, as it is a sedative. They help with excess itching if present.
- Avoid stagnation of moisture and sweat. Make sure to wipe out in case of sweating and air the area well under a fan from time to time. They help with the current lesions and for preventing recurrences.
- Make sure to wear loose clothing and underwear.
- Make sure to wash your used belongings and dry them under the sun daily.
- Do not share your belongings, and make sure your family members or friends, if affected, need treatment to prevent recurrences.
- Take capsule Becosules once daily after breakfast for one month to maintain immunity.
However, please consult a specialist doctor, discuss with them and start taking the medications with their consent.
