I have discoloration and swelling on the glans penis for almost a year now. I have tried different creams prescribed by a dermatologist nothing has helped. Also, my primary did a urine culture and it was normal. Please help.

I saw the pictures (attachment removed to protect patient identity). This patch is normal there are no issues with it. It is melanin pigmentation and it is completely no issues and will not cause issues in the future. I am assuring you for that.

What about the irritation on the tip and the little black bumps under the surface of the skin?

You have albinism at the base of the penis which is discoloration and this is a congenital problem (problem from birth). It is completely alright and no need for treatment. The only thing is the irritation on the tip of the penis that can be allergic due to soap you are using or body wash you are using. I do not think you have any infection until you are sexually active and had unprotected intercourse, then you need further investigations and work up otherwise, just use a mild soap like baby soap. Hydrate well with two to three liters of water daily.

Same symptoms don't mean you have the same problem. Consult a doctor now!
