Is my hair thinning due to baldness or fungal infection?

They all differ in presentation and can coexist. Tinea capitis is not common in adults, though it can occur, but most common in children before the age of 15 and usually presents as patchy hair loss with scaling, redness, or itching.

In adults, seborrheic dermatitis is more common and can contribute to hair shedding, but rarely causes complete bald patches; it causes flakes, bumps, or redness in the scalp and sometimes discharge, while in androgenetic alopecia, there is just hair fall with receding hair line from side and front without any pain, redness, itching, or bumps in the head.

You may have two conditions existing together to guide you better if you are having both or one, I would appreciate it if you could share a clear picture of your scalp from front and side along with details like if you are experiencing any itching, bumps in head, excess dandruff or flaking, any redness or fluid in the scalp, along with hair fall, so that I can guide you better.

Now, regarding your question, yes, you can use Ketoconazole shampoo safely alongside Minoxidil to treat any fungal infection or scalp inflammation, as we do pair the two in many patients having seborrheic dermatitis with androgenetic alopecia. Minoxidil helps stimulate hair growth in androgenetic alopecia, and stopping it suddenly can cause rebound shedding of the hair it has been maintaining, which is normal and temporary but distressing.

So I would recommend you continue using both Minoxidil and Ketoconazole shampoo together, as it is safe to use both and they do not lower each other’s efficacy. Just use a conditioner afterwards, as Ketoconazole is drying.
