Hello doctor,

My brother has cardiac valve disease, and he is currently still in critical condition after recovering from a cardiac arrest. The doctor told us that his blood pressure was too low (average 40mmHg) and was too dangerous to do surgery. However, it is also not safe if we just wait for him to get better. What should we do? Please help.

If you mean from a valve, a blockage in the artery, then treatment is immediate angioplasty. This will open arteries, and blood supply to heart muscles will be restored, and blood pressure may improve. If you mean from the valve, the real valve like the mitral or aortic valve, in case of severe valve narrowing, the treatment of choice is valve replacement or repair, but this can only be done when the patient stabilizes. For that, intravenous fluids and medicines to increase blood pressure, etc, are needed to support the patient's blood pressure and heart rate. A ventilator may be needed in both cases to support the lungs. All of the above treatments also depend upon age and other concomitant illnesses like liver, kidney, and brain function.
