Patient's Query
Hello doctor,
My 68-year-old dad, who has heart failure, has been severely affected by norovirus. He is unable to keep Lasix down, and his legs are severely swollen. He has gained 12 pounds in just two days from IV (intravenous) fluids but is still vomiting. His oxygen dropped to 88%, and he is breathing heavily. His digoxin level is dangerously high at 3.1, and his potassium is at 5.8. His pacemaker is showing irregular rhythms.
Please help.
Thank you.
Hello,
Welcome to icliniq.com.
I have read your query and can understand your concern.
Managing norovirus in patients with heart failure is especially challenging. The infection causes significant fluid loss, but intravenous fluids are necessary, leading to fluid retention and swelling, particularly in the legs. The reported 12-pound weight gain over two days and concerning oxygen saturation of 88% are worrisome, especially with concurrent hyperkalemia (high potassium) and elevated digoxin levels.
I strongly recommend taking your father to the emergency department immediately for treatment of the hyperkalemia and a full evaluation of his fluid status. Given the complexity of his condition, home management will likely not suffice. A healthcare team can provide the comprehensive care and interventions he needs.
If his ECG (electrocardiogram) is showing irregular rhythms, it is crucial to have his pacemaker checked and undergo a device interrogation as soon as possible.
I hope this answers your query.
Please let me know if I can assist you further.
Thank you.
