Does heavy bleeding during pregnancy indicate miscarriage?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am currently 11 weeks pregnant and have fibroids. I have been experiencing on-and-off spotting since around six weeks. However, today the bleeding has become heavy enough to soak a sanitary pad. I have had previous miscarriages, so I am very concerned.

What should I do?

Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I have gone through your query and understand your concern.

I am really sorry you are going through this. Bleeding that fills a sanitary pad during pregnancy, especially with a history of previous miscarriages and known fibroids, is an urgent concern.

Here is what you need to do immediately:

  1. Go to the emergency room or contact your gynecologist right away.

  2. Heavy bleeding in pregnancy (filling a pad) could be a sign of threatened miscarriage, placental problems, or complications from fibroids.

Your gynecologist may want to do:

  1. An urgent ultrasound to check for fetal heartbeat and placental position.

  2. A physical examination.

  3. Bloodwork (like hCG (human chorionic gonadotropin) levels, hemoglobin).

Monitor associated symptoms. If you are experiencing any of the following, seek care immediately:

  1. Cramping or abdominal pain.

  2. Clots or tissue passing.

  3. Dizziness or fainting.

  4. Fever or chills.

Rest and avoid certain activities. Until you are evaluated, avoid:

  1. Intercourse.

  2. Heavy lifting.

  3. Vaginal examination (unless directed by a provider).

Important context for your case:

  1. Fibroids can increase the risk of bleeding, miscarriage, and other pregnancy complications.

  2. Given your previous miscarriages, this should be treated as a high priority.

  3. The fact that spotting started at six weeks and has worsened now may signal a change that needs immediate attention.

Please do not wait — seek medical care right now. If you are unable to reach your gynecologist quickly, go to the nearest emergency department. Early intervention can sometimes preserve the pregnancy or address risks to your health. Let me know how I can support you further.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.
