This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have been experiencing extremely heavy bleeding with large blood clots and intense, cramping pain similar to labor for the past three days. I am soaking through a pad every hour, and the bleeding seems to be getting worse. Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I read your query and can understand your concern.

I am sorry you are going through this. What you are describing is heavy bleeding that soaks through a pad every hour, large blood clots, and severe cramping pain similar to labor, which is not normal and could indicate a medical emergency, especially since it has persisted for several days and is worsening.

Several serious conditions could be causing this, including:

  1. Miscarriage, if there is any chance of pregnancy, even if you were not aware of it.

  2. Uterine fibroids or polyps can cause sudden or heavy bleeding.

  3. Hormonal imbalance, potentially triggered by stopping medications like Lexapro (uncommon, but possible).

  4. Endometrial or uterine abnormalities, such as endometrial hyperplasia or, in rare cases, uterine cancer, especially if you are over the age of 35.

  5. Ovarian cyst rupture or other acute gynecologic issues.

Right now, the most important step is to get medical attention immediately. Please go to the Emergency Room or a gynecology-capable urgent care center as soon as possible.

You may be at risk for:

    1. Anemia or hypovolemic shock can lead to symptoms like weakness, dizziness, shortness of breath, or fainting.

    2. Continued blood loss may require urgent treatment, such as IV fluids, medications to control bleeding, or a procedure like a dilation and curettage (D&C).

This situation is unsafe to monitor at home, especially since it is progressing. Medical professionals can assess the cause and begin treatment right away to stabilize you and prevent further complications.

If you would like, I can also walk you through what to expect during your ER visit, including tests like ultrasounds, bloodwork, and treatment options. Please do not delay. Your health and safety come first.

      I hope this helps.

Kindly revert so I can assist you further.

      Thank you.
