Hi doctor,
I am a 32-year-old female recently diagnosed as a carrier for hemophilia A. I never thought it would affect me directly, but I am now pregnant (14 weeks), and I am starting to get anxious. My OB says we will need to monitor the baby closely, but I am scared about bleeding risks during delivery and whether my baby could inherit this. Are there special precautions I need to take now or later during labor? I have had a few heavy bleeds after dental work and my periods have always been heavy too. Should I be seeing a haematologist throughout pregnancy? What kind of delivery is safest? I just want to understand everything clearly and in simple terms.

I completely understand your anxiety, but you are in good hands. Here is the plan:
Hematology consult: Essential now—they will monitor your factor levels and plan for delivery.
Baby’s risk: 5 percent chance if male; cord blood testing at birth confirms.
Delivery: Vaginal birth is usually safe. Avoid forceps or vacuum. Cesarean only if needed.
Precautions: - Get factor VIII levels checked before delivery. May need IV factor concentrate during labor. Avoid epidurals if levels are low.
Postpartum: Watch for heavy bleeding; Tranexamic acid helps.
