How can I stay active with hemophilia without more bleeds?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I wanted to talk about my bleeding from the injury. It started after something as small as bumping my knee, which honestly scared me. I infused my factor right away, and the bleeding stopped, but the joint is still sore.

  1. I am trying to stay active because I know it is important, but I am constantly worried that exercising might trigger another bleed. Are there safer ways to strengthen my joints without putting myself at risk?

  2. I also wanted to ask about the newer long-acting factor treatments. I have heard they do not require infusions as often. Would those be an option for me? It would make day-to-day life so much easier.

  3. Also, I get anxious when I travel or even go out for long periods because I am always thinking about whether I have everything I need. I would really like to feel more independent and not constantly worry about getting hurt.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have read your query and can understand your concern.

Living with hemophilia and dealing with bleeds, even from something as small as a bump, can be both painful and emotionally stressful. You acted very responsibly by infusing your factor promptly, which likely prevented further joint damage. The soreness you are still feeling is normal; joint tissues take time to heal, and mild inflammation can persist even after the bleeding stops. Rest is important initially, but gentle movement and gradual physiotherapy help restore strength and prevent stiffness.

For exercise, low-impact activities are the safest and most effective for protecting your joints. Swimming, cycling, and light resistance exercises under a physiotherapist’s guidance are excellent choices. Avoid high-impact or contact sports that increase your risk of falls or collisions. A hemophilia-trained physiotherapist can create a program tailored to your joints and activity level, helping you stay active safely.

Regarding long-acting factor products, these are now widely available and can significantly reduce the frequency of infusions. Extended half-life factor VIII and IX products, such as Efmoroctocog alfa (Elocta) or Eftrenonacog alfa (Alprolix), last longer in the body, so infusions may be needed only once every few days or weekly, depending on your type and response.

Non-factor therapies like Emicizumab (Hemlibra) are given as simple subcutaneous injections and offer strong protection against bleeds. These options can improve independence and make daily activities and travel much less stressful.

Feeling anxious about injuries or always needing to carry supplies is completely understandable. With modern treatments and a personalized preventive plan, many patients live active, fulfilling lives with fewer interruptions from bleeds. Your hematologist can help you choose the best therapy to give you more freedom, confidence, and peace of mind. You deserve to live without constant fear, and with the right plan, that goal is absolutely achievable.

I hope this answers your query.

Please let me know if I can assist you further.

Thank you.
