Could inflammation in genitals and dry skin indicate herpes?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have noticed inflammation in my genital area, which I just noticed today. I masturbated the day before and have been experiencing dry skin in that area recently. I am concerned that this might be herpes. I will attach a picture for your reference; one of the pictures was taken at home using a microscope. Please help.

Thank you.

Hello,

Welcome to Icliniq.com.

I have read your query and understand your concern. I have reviewed the pictures you attached (attachments removed to protect the patient’s identity), it appears to be genital wart (verrucae) with penile melanosis. I suggest you follow the below mentioned instructions:

  1. As penile skin is very sensitive skin, apply Cidofovir (anti-wart cream) one percent gel at night for three to seven days.

  2. Wash the area with a cleanser containing purified Water, Cetearyl alcohol, 1, 3 Butylene glycol, Maltooligosyl glucoside, Hydrogenated starch hydrolysate, Cetostearyl alcohol, Sodium lauroyl sarcosinate, Phenova, Acrylic acid/C10-C30 Alkyl Acrylate copolymers, Triethanolamine or a no soap cleanser daily for 20 days.

  3. Avoid scratching and rubbing of the lesion.

  4. Decontaminate surroundings.

  5. Other treatment options are electrodessication thermal cautery, or Co2 laser.

I hope this helps. Revert in case of other queries.

Thank you.

Patient's Query

Hi doctor,

Thank you for your reply.

However, this is a skin wound and not a wart. It is similar to when you fall and injure your skin. There are no other accompanying symptoms. The discomfort is only experienced when I shower. Please help.

Thank you.

Hello,

Welcome back to icliniq.com.

I read your query and can understand your concern.

Based on the closely examined of recently uploaded image (attachments removed to protect the patient's identity), the condition seems to be a minor erosive lesion likely caused by scratching. I suggest applying Fucidin cream (20 mg of fusidic acid) twice daily for ten days. To prevent any potential secondary bacterial infection please refrain from scratching or rubbing the affected area to prevent any possible secondary bacterial infection. It is important to avoid friction or occlusion as well.

Could you provide additional information on the following aspects:

  1. Is there any oozing?

  2. Do you experience a burning sensation?

  3. Have you noticed any tingling?

  4. Is this episode recurrent?

  5. Is the area painful or tender to touch?

I hope this helps. Please revert in case of further queries.

Thank you.

Patient's Query

Hi doctor,

Thank you for your reply.

There is slight pain upon touching the area. The issue does not recur. There is a mild discomfort but no sensation of burning. I am experiencing slight itching and a minor sensation of oozing. Should I be concerned?

Hello,

Welcome back to icliniq.com.

I read your query and can understand your concern.

There is no need to be concerned. The issue is a minor erosion caused by scratching due to a mild itch. It is important to avoid scratching. For treatment, I suggest the following:

  1. Apply Mupirocin cream twice daily to the affected area for ten days.

  2. Make use of a gentle cleanser to wash the genital area daily.

  3. After washing, gently pat the area dry using a soft towel.

  4. When applying the cream, ensure your hands or a cotton tip applicator are clean.

  5. With proper hygiene practices and preventive measures, it should take a few weeks for the issue to fade and heal.

  6. I suggest you to practice safe sex.

Could you please provide the following information:

  1. Are you sexually active?

  2. Have you engaged in unprotected sex?

  3. Has your partner experienced similar complaints?

  4. What is your marital status?

I hope this helps. Please revert in case of further queries.

Thank you.
