Hello doctor,

I have been diagnosed with intermittent claudication in my stomach, which causes agonising pain in my hip, thigh, and calf, and I am waiting to see a surgeon, but I am also feeling breathless and often feel like I am struggling to breathe at all. Is this related or a separate issue?

Claudication, especially when affecting the abdomen and lower limbs, often stems from significant atherosclerosis, which is a systemic condition and can also affect the coronary arteries (leading to ischemic heart disease) and the vessels supplying the lungs. Breathlessness may indicate heart involvement, such as angina equivalent, heart failure, or possibly reduced oxygen delivery due to compromised circulation. It could also point to a separate issue like pulmonary disease or even anemia, but given your known vascular disease and lack of treatment so far, I would treat this as potentially cardiac in origin until proven otherwise.

You need a full cardiovascular workup, including ECG (electrocardiogram), echocardiogram, and potentially cardiac stress testing or CT (computed tomography) angiography to assess for coronary artery disease.

These symptoms should not be ignored, especially if they are worsening or affecting your daily activities.
