Hello doctor,

I am a 26-year-old woman, dealing with persistent acne since my teenage years. I have tried OTC creams, Benzoyl Peroxide, and even oral Doxycycline before. Breakouts worsen around my periods, sometimes with painful cystic lesions on my jawline. My mother had similar adult acne.

There is no PCOS on ultrasound, and my hormones are normal. I am currently using a Retinoid cream, but my skin gets too dry. I am scared of scarring. My dermatologist suggested Isotretinoin, but I am hesitant due to the side effects and the need for liver monitoring. Are there safe, long-term skincare routines that actually control hormonal acne?
