Hi doctor,
My friend has HSV-1 on his penis. Is it safe for me to perform anal sex on him as a gay man? I would be topping.

Since your partner has herpes simplex virus (HSV-1) and you want to continue with anal sex with him, I do not think it will be safe to do so because HSV is a major cause of anorectal cancer. So if you go on to have sex with him via anal, you are putting yourself at a greater health risk.
So the only possible way anal sex could be possible is for him to wear a double condom and let him treat himself and be free from the HSV before he can resume unprotected anal sex.
