I had swelling in my left scrotum. Ultrasound confirms bilateral hydrocele. The size is small. Will it disappear on its own?

I understand your concern. Hydrocele happens due to a mismatch in fluid formation and absorption within the sac around the testis. Once fluid produced is more than absorbed, it collects, and the size gradually increases. Some hydroceles may remain the same size. Sometimes, if it occurs due to some inflammation in the testis, it may reduce or disappear if the inflammation is treated. In your case, if the size is small or not bothersome, we can wait. However, if the size is more or it is causing cosmetic issues or heaviness, we need to operate and remove one or both side hydroceles. You can send me an ultrasound report here. Till then, use scrotal support or tight-fitting underwear.

I understand your concern. Medicines do not work for hydrocele. Sometimes size may vary. Also, needle aspiration is not advised as fluid collects again. Normal activity can be done.

I suggest you get a urine routine and urine culture test done along with CBC (complete blood count) and serum creatinine. Do you have any cough, cold, urine-burning sensation, or other symptoms? I suggest you take a tablet of Nitrofurantoin 100 mg twice daily for seven days for urine infection after giving a urine sample and a tablet of Crocin (Paracetamol) 500 mg for fever three times a day or as needed. Kindly let me know the reports.

I have attached CBC and creatine report. I am waiting for the urine test reports. I have no fever, but my TLC seems to be high due to hydrocele. I checked my fasting glucose it was 130 mg/dL. I have no other symptoms.

I have seen the attached reports (attachment removed to protect the patient's identity). Yes, there is an infection, most likely in hydrocele. Repeat TC (total count) after five to seven days, and let me know when the urine culture report comes. If the fever does not subside or the fever comes again, or there is any weakness, get checked by your doctor if it requires admission and antibiotic injections.
