I am a 50-year-old female. I have had diabetes for a year. I want to know about hyperglycemia, as I have already experienced it before. Please briefly explain what hyperglycemia is and its common causes. What are the typical symptoms of hyperglycemia? What are the recommended strategies for managing high blood sugar levels? Are there any proactive measures that can be taken to prevent the occurrence of hyperglycemia?

Well, hyperglycemia is high levels of glucose in your blood when your body is unable to produce enough amount of insulin to maintain the glucose level in range. Insulin is produced by the organ called pancreas in our body. It could be genetic or acquired. There are mainly two types. There are other types as well but it is difficult to label someone in the start of diagnosis. Typical symptoms are osmotic symptoms (increased thirst and urination, fatigue, lethargy, tiredness, recurrent infections, and weight loss). Recommended strategies depends how advance your condition is. Usually we start from lifestyle measures and oral hypoglycemic medications. Then we titrate the treatment accordingly. If some is diagnosed to be in prediabetic state, the only preventable measure is change in lifestyle. There are some pancreas transplant options for type one diabetes but there is a certain criteria and it has to be quite advanced stage. It will be of importance if we take a detail history from you in regards to know about your family history, other medical problems, medications, and previous investigations done.

Investigations to be done

Complete blood count, renal functions, liver functions, HbA1c, ECG, bone profile, and urine for ACR (urine albumin to creatinine ratio).

Treatment plan

1.Oral hypoglycemic drugs. Consider multiple drugs if not controlled with one drug. 2. Consider Insulin therapy if uncontrolled glucose is present while being on oral medication. 3. Annual retinal and foot screening. 4. Dietitian input is needed for a better understanding of food choices. 5. Regular BP (blood pressure) and glucose monitoring.

Preventive measures

1. Life style measure (exercise and dietary measures). 2. Weight loss.

Regarding follow up

Initially six months follow up and then one yearly.
