Hi doctor,

My friend has hypertension and diabetes mellitus. He has body pain and poor appetite, and after each meal, he goes to the toilet to pass stools. About 15 days back, he had a fever, and after that, he developed this problem. He also has very poor sleep. His BP is 113/69 mmHg, and his pulse is 70 BPM. He currently takes Telma ND-5 mg (Telmisartan 5 mg), Vidader MET-50mg/500 mg (Metformin 500 mg and Vildagliptin 50 mg), Rozutin 10 mg (Rosuvastatin), Clocit-5 (Clonazepam 0.5 mg and Escitalopram oxalate 5 mg), Gaspunch (activated charcoal 250 mg and Simethicone 80 mg), and B-rest- 0.5 mg (Alprazolam). The doctor recommended B-rest only once, but he is taking three tablets without any advice). I am attaching his present blood test results, which shows many abnormalities.

I saw the reports (attachments are removed to protect the patient’s identity). He has post-viral syndrome fatigue, eats more vegetables and fruits, and takes rests. The most important problem is acute kidney injury creatinine going up. He should see a doctor nearby, preferably a nephrologist. For now, stop the tablet Vidader MET-50mg/500 mg (Metformin 500 mg and Vildagliptin 50 mg) instead take the tablet Ondero 5 mg (Linagliptin) daily. Ask him to stop Telma (Telmisartan 40 mg) and take Amlong 5 mg (Amlodipine) daily. Reduce the tablet containing Clocit (Clonazepam 0.5 mg and Escitalopram Oxalate 5 mg) and use B-rest (Alprazolam 0.5 mg). All the other things can be sorted out slowly. I suggest he repeat creatinine and electrolytes after a week and see a nephrologist as soon as possible.

We have Ondero 5 mg (Linagliptin) instead of the tablet Vidader MET-50mg/500 mg (Metformin 500 mg and Vildagliptin 50 mg). How many times and what time should Ondero 5 mg (Linagliptin) be taken? Is it before or after food, and how many times a day?

Tablet Ondero 5 mg (Linagliptin) should be taken once daily in the morning after food. Once he recovers, we can deal with triglycerides and elevated urine albumin.
