Hello doctor,

I have infertility due to hypogonadotropic hypogonadism. I have a problem with low hormone levels. I have been taking hMG 150 and hCG 5000 twice a week for the last year. Still, my hormone level of LH, FSH, and testosterone is not increased, and my sperm count is zero. Please let me know what needs to be done and if any other treatment option is available. I am diabetic, and it is under control. I have found this problem before marriage. My local endocrinology doctor suggested taking a Testosterone injection while trying for the baby. He stopped that injection and gave me hMG 150 and hCG 5000 twice a week. I have done a test on the Y chromosome test, and it is normal and took an MRI of the pituitary, and it is normal.

Please answer the following questions to know more.

1. Did you go through normal puberty and develop hypogonadotropic hypogonadism later in your life?

2. Or did you not go through puberty and start testosterone injection to start puberty?

3. What is the size of your testicles?

If you had hypogonadotropic hypogonadism before puberty, then the chances of having adequate sperm count are low, despite best efforts. One additional treatment option available in foreign countries which may not be available here is the GNRH (Gonadotropin-releasing hormone) pump. I am not sure if increasing hCG (human chorionic gonadotropin) and hMG (Menotrophin) to three times weekly will be of any added benefit, but I can try.

I did not have normal puberty and started through injection. I was planning for IVF, but my hormone level did not increase after the hMG and hCG injection. I was injecting in the buttock location using a 2.5 ml syringe, and now my doctor suggested putting the injection in the stomach location using an insulin needle. I am using Lupi hCG 5000 and GMH HP 150. Please let me know whether this is fine. Please let me know if any other option is there to increase hormone level, or I can add an FSH injection once weekly and two hMG. Please help me with this. I am hoping to get some sperm for the IVF. Please find the scan report.

Do increase the hCG and hMG to three times a week. Injecting using an insulin syringe is fine. Check testosterone level in one month and recheck sperm count in six months.
