Can hypothalamic obesity affect my fertility?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am 31 and was recently told that my unexplained weight gain might be due to hypothalamic obesity. Even though I eat carefully and exercise regularly, I keep gaining weight. This has started to affect my confidence and has also made my menstrual cycles less regular.

In the past few months, my periods have become more irregular and sometimes I skip them for two or three months. I am worried that this might affect my fertility later on. I want to know if hypothalamic obesity can impact ovulation and my chances of getting pregnant, either naturally or with IVF.

  • I would also like to know if weight-loss surgery is ever recommended for people with hypothalamic obesity. If so, does it improve reproductive health?

  • I am also concerned about the hormonal side of this condition. Could taking birth control pills make weight gain worse, or could they help regulate my periods?

  • I am worried about how hypothalamic obesity might affect pregnancy. Does it raise the risk of problems like gestational diabetes, or does it tend to get worse during pregnancy?

  • Are there any treatments specifically for hypothalamic obesity, or is it usually managed with lifestyle changes and hormone therapy?

Please help.

Thank you for your time and help.

Hello,

Welcome back to icliniq.com

I understand your concerns.

Unexplained weight gain is not always caused by hypothalamic obesity. It can have many causes, including hormonal imbalances like thyroid problems. It is important to look at all possible factors before making a diagnosis.

Hypothalamic obesity is usually associated with injury, dysfunction, or a mass affecting the hypothalamus, a part of the brain responsible for regulating appetite, metabolism, and hormones. Common symptoms may include significant weight gain that is difficult to control, along with issues such as sleep disturbances.

Since you are also experiencing irregular menstrual cycles, further evaluation is important. To better understand the underlying cause, I would recommend the following investigations:

  • Complete blood count (CBC).

  • Thyroid function tests (TFT).

  • Fasting blood sugar.

  • Serum LH and FSH levels.

  • Ultrasound of the whole abdomen with pelvis.

These tests will help assess for hormonal imbalances, metabolic conditions, and gynecological causes that may be contributing to your symptoms. Once the reports are available, we will be in a better position to determine the diagnosis and discuss the most appropriate treatment plan.

Even if hypothalamic obesity is confirmed, management typically focuses on maintaining a healthy diet, regular exercise, and hormone therapy when indicated.

I hope this answers your questions.

Please feel free to follow up whenever needed.

Thank you.
