How to manage weight gain after brain surgery in a 24-year-old?

Hi doctor,

I am 24 years old. I gained a huge weight after brain surgery for craniopharyngioma three years ago. The doctor said it is hypothalamic obesity. No matter how much I diet or exercise, my weight keeps increasing. I also feel sleepy all day and have hormonal issues. People think I am just lazy, but it is not in my control. Are there any new treatments, like medicines or injections, that can actually reduce weight in this condition? Or is bariatric surgery an option here?

Hypothalamic obesity after craniopharyngioma surgery is unfortunately very difficult to manage because the brain’s appetite and energy balance centers are affected, making diet and exercise alone far less effective than in ordinary obesity.

The constant weight gain, excessive sleepiness, and hormonal problems you describe are common in this condition and not your fault. Treatment usually requires a combination of approaches:

  1. Strict hormonal replacement (thyroid, cortisol, sex hormones as needed).

  2. Careful nutrition and physical activity.

  3. Medications that act on appetite and metabolism.

Newer options such as GLP-1 (glucagon-like peptide-1) receptor agonists (like Semaglutide) and other anti-obesity drugs are being studied and may help in selected patients, though their effectiveness in hypothalamic obesity is variable.
