Can Opdivo help someone aged 64 with distant spread?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I am asking about my father, who is a 64-year-old male and has metastatic urothelial carcinoma with spread outside the bladder. After hearing the word “metastatic,” our whole family has been trying to understand what realistic treatment options still exist.

His oncologist recently mentioned Opdivo (Nivolumab) immunotherapy, and we are hopeful but also trying to stay realistic about expectations. He has already been through treatment before, and we do not know whether immunotherapy works differently when cancer has spread.

For people with metastatic urothelial carcinoma, can Opdivo still provide a meaningful response or disease control when there is distant spread?

Please help.

Thank you.

Hi,

Welcome to icliniq.com.

Thank you for sharing your concern.

Yes, Opdivo (Nivolumab) can still play an important role in metastatic urothelial carcinoma. Once bladder cancer has spread outside the bladder, the goal is usually to:

  1. Control the disease.
  2. Prolong survival.
  3. Maintain quality of life.
  4. In some patients, a meaningful and durable response is achieved.

Immunotherapy differs from chemotherapy. Unlike treatments that target cancer cells directly, it helps the patient's immune system recognize cancer cells. Some people have experienced strong and durable responses to this treatment despite having metastatic cancer.

That said, responses vary from person to person. Some patients have substantial tumor shrinkage or prolonged disease stability, while others may have more limited benefit. Factors such as:

  1. Overall health.
  2. Extent of disease.
  3. Prior treatments.
  4. Tumor characteristics.

All these can influence outcomes.

The important point is that metastatic urothelial carcinoma is not considered untreatable simply because it has spread. Modern treatments, including immunotherapy, have significantly expanded the options available and can provide meaningful disease control for many patients.

I would encourage focusing on the goals of treatment and how your father is doing clinically rather than the word "metastatic" alone. Many patients continue treatment for extended periods while maintaining a good quality of life.

I hope it helped with the query.

Please let me know if you have any more questions on your mind.

I would be happy to guide you.

Thank you.
