Is positive indirect Coomb's test in pregnancy serious?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My blood group is A-negative, and my husband's blood group is O-positive. I am six months pregnant. The indirect Coombs test is negative as of now. The doctor advised an option of an injection at 28 weeks, but they did not agree to take it. Please tell me the following:

  1. What happens if the ICT is positive?
  2. Does it affect my baby? I am really confused and worried at this moment.
  3. The doctor told me my baby might be affected by jaundice or anemia. What to do?

Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I went through your query and understood your concern.

If you are blood group negative and your husband is positive, there is a chance of sensitization if bleeding occurs during pregnancy.

So, as a routine (previously) at 28 weeks, ICT (indirect Coomb's test) is done, and if it is negative (that means sensitization has not occurred), an injection (anti-D immunoglobulin) is given. This is, however, not a routine practice again; that is probably why your doctor told you it is optional.

Also, it is only babies who get sensitized during pregnancy that develop jaundice and anemia. So, for now, you and your baby are fine. Just avoid things that can cause bleeding in pregnancy, like trauma to the abdomen. However, note that delivery causes bleeding, and you would need to take the injection within 72 hours of delivery.

Hope it helps.

Please revert in case of further queries.

Thank you.
